Incident:

On [REDACTED], Junior Researchers [REDACTED] [REDACTED] and [REDACTED] [REDACTED] signed out AI55 "Radio Loop Antenna" from its cell in the Panopticon using a falsified research order (see attached evidence form).

AI-55's altered effect was intentionally activated in the Research Sector, creating a new Threshold (designation pending). Investigators belief the offending party was attempting to create a passage leading outside the Oldest House with the intention of deserting their posts.

Sentencing

The offending party has been permanently stripped of all security clearance. Criminal sentencing has been suspended due to emergency conditions. Offenders will be relegated to menial duties, under supervision, until conditions allow for sentencing.

Trivia

• Per Hank's narration on entering the Frequency Shift job area, the incursion of the pink Exogenic Mass into the Oldest House was the direct result of this incident.
• The Radio Loop Antenna can be seen in a broken containment room on the second floor, above the third area's emitter array.
